Questions To Ask - Please feel free to come to your appointment with a list of questions you would like your Provider to answer. Your health insurance card(s) and any required authorization, pre-certification, or referral forms. Any pertinent medical information, records, or test results given to you by another Physician. A list of any medications you have been taking and/or the medicines in their prescription containers. A list of any known food and/or drug allergies. If your treatment is covered by Workers' Compensation or NYS No-Fault insurance, please make us aware of this when you schedule your appointment and bring the appropriate authorization information from your employer or auto insurance company. Financial resources (cash, personal check, money order, VISA, Discover, MasterCard, and American Express are accepted) to cover co-payments or to make payments on your account at time of service.
PRESCRIPTIONS - Our office uses Surescripts for e-prescribing, a web based solution that exchanges prescription information electronically with your pharmacy. Surescripts allows two way communication between CVS, Kinney, Rite-Aid, Wayne's, Walgreens, Wal-Mart, Wegmans and mail-order pharmacies to allow us to send prescriptions electronically.
*We kindly ask that you allow us three business days to accommodate your prescription refill requests*
We provide service for all of our patients regardless of their ability to pay. *Discounted/sliding fee scale payment options are available for those who qualify.
No patient will be denied access to care due to the inability to pay, having coverage under Medicare/Medicaid/CHIP, race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.
